This program is tailored to address your individual needs
for risk factor management.

The goals of the Program are to:

  • Introduce you to heart healthy exercise to help you manage your cardiac risk factors at home
  • Provide you with the knowledge to exercise safely
  • Tailor the program to your individual needs for risk factor management
  • Maximize the effectiveness of each workout
  • Improve your overall fitness level
  • Lower your chances of developing a heart attack or stroke in the future.

The Program includes:

Regular monthly visits with CHIP physician

Pre and Post Exercise stress test

Pre and Post Blood Tests (Cholesterol, Glucose, TSH, Liver Function)

Pre and Post Computerized Cardiovascular Risk Profile

Psychosocial Assessment Profile

4 Private training sessions with an exercise physiologist (3.5 hrs over 3 months)

Telephone follow-ups at 3rd, 7th, 9th, and 11th week of program

Individualized exercise prescription package

Record of results to referring physician

Follow-up for 1 year upon completion of program
