4 Month Multidisciplinary Program

The Diabetes Programs include a 4 month program designed to help individuals with insulin resistance syndrome or with diabetes to learn to exercise correctly and adopt healthy dietary choices.
At CHIP, we seek to provide you with the tools to exercise safely and effectively in an environment that includes support and counseling.
Embarking on this program may be the first step in a long-term change to a healthy lifestyle

The Program includes:

On Site Medical Evaluation and Pre and Post Exercise Stress Test to determine the intensity at which you can exercise safely.
Computerized Cardiovascular Risk Profile prior to the start of the program and at the end to assess your risk of developing cardiovascular disease.
Individualized Personal Training Session with an exercise physiologist who will design a safe and effective exercise program for you.
36 Supervised Exercise Sessions with an exercise physiologist.
Blood Sugar Monitoring to measure glucose levels before and after each exercise session.
Development of an Independent Exercise Strategy to teach you how to exercise on your own when the program finishes.
Dietician Sessions on-site once a month over the six-month program. (a total of 2.5 hours)
Regular On-site Visits with a Physician
Pre and Post Blood Tests - Cholesterol, Glucose, TSH, Liver Function, Hemoglobin A1c, Creatinine
