Developing a Long Term Exercise Strategy

 

This program includes an additional series of supervised sessions designed to complement the Prevention & Rehabilitation Program. This program will help to ease the transition between supervised and independent exercise. The objective is to ensure continued success & compliance with lifelong risk factor management.

 

Strength Training

It will also include more individualized resistance training and flexibility exercises. These exercises will be geared towards improving your ability to perform your activities of daily living.

  • 24 Supervised exercise sessions 2—3 times per week over 3 months
  • Follow- up with a CHIP physician
  • Blood tests (Lipid Profile)
  • On going education
  • Final Exercise Stress Test with updated Target Heart Rate
  • Long-term exercise plan upon completion

 

Annual Health Check
  • Update your cardiovascular program and adjust the training heart rate zone to ensure optimal training benefits.
  • Obtain and / or update your strength training program to ensure a safe and appropriate progression of exercises and that they are meeting your personal needs for daily living.
  • Update your assessments by CHIP physicians, exercise physiologists, dieticians and psychologists as needeThe CHIP Graduate health Check Program includes:

Annual Medical Evaluation and Exercise Stress Test

  • A graded exercise test to confirm the intensity at which you can exercise safely using your Target Heart Rate
  • An update of your Computerized Cardiovascular Risk Assessmen

Personal Training Sessions
6 hours of individualized sessions to review and update your cardiovascular and strength training programs throughout the year
